What is the Access to Resources in the Community (ARC) study?
Many health and social community resources exist to promote well-being and healthy active living, and prevent or manage chronic disease. Consultations with health planners, providers, patients, community members, and other stakeholders in our region have identified that primary care providers are often unaware of the extent of community resources available and do not know where to find such information. Additionally, patients seeking health services from their family physician often do not know what resources exist in the community that may be beneficial. The ARC study will look at how helpful Navigators are to link patients at family doctors' offices to community resources.
The objective of the ARC Study is to:
Increase equitable access to community-based resources for socially complex primary care patients, including Franco-Ontarians living in minority situations.
Key elements of ARC include:

Promote awareness among primary care providers and patients about available community resources;

Develop processes to support providers to refer to these resources;

Integrate a Navigator within primary care practices to assist patients to reach community resources to help address their needs such as mental health, physical activity, transportation, healthy eating, and parenting and family support.
